IP Address Request Form
Please fill out the form below as completely as you can. IP address for a server machine will be translated into a visible number on the Internet. The machine name will be hostname.yoursite.sunyeoc.org.

To determine your Mac Address (Physical Address), type ipconfig /all from the DOS prompt. If you cannot, please fill out the form anyway and we will determine it for you at a later date.

Please be aware that real IP numbers are limited and the request may be turned down based on IP address availability.
